This is to be filled out by the existing team captain or on behalf of someone authorized to make changes for their team. This form is only for the initiation of changing the team captain for the specified team.
Upon submission of this form two emails will be sent out. One will go to the website support staff notifying them of a pending team manager change request. The second email will go to the new team manager and will contain instructions that will allow them to complete the team manager change. Please follow any instructions given below and fill out all the required fields.
